Job description

Are you ready to make a meaningful difference in the lives of young individuals and deliver positive person centred outcomes for people with complex support needs who rely on our services.

You will need to be enthusiastic, have great people skills, and a positive and proactive approach to your role, supporting individuals with Learning Disabilities, Autism, Mental Health and complex needs.

Where you’ll be working:

Devizes and Surrounding Area

We are seeking a caring, patient and motivated Support Worker to join our Outreach Team in Wiltshire.

We support a number of clients in the local area who live in their own homes.

You will help them live as independently as possible, accessing community, building life skills, and achieving personal goals.

You will work in a person‑centred way, promoting choice, dignity and inclusion at all times.

What is required from you?

  • A minimum of 12 months right to work
  • Full‑time, part‑time and bank shifts available

Supporting – Activities of daily living including social activities, personal care, routines, planning and preparation of meals and budgeting.

Communicating – effectively communicate using people’s chosen methods.

Engaging – promoting and motivating people we support to enhance their quality of life.

Specialising – supporting people who may need positive behavioural support approaches.

Recording – keeping accurate records and reporting changes in health or circumstances of service users to management.

Team player – working as part of a team, as well as being able to work using your own initiative.
